HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can gain access to this information. Please review it carefully. Raeven Health, LLC is committed to safeguarding your protected health information ("PHI") in accordance with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and its implementing regulations, as well as applicable state privacy laws.
Our Commitment to Your Privacy
Raeven Health maintains administrative, physical, and technical safeguards designed to protect the confidentiality, integrity, and availability of your protected health information. This includes encryption of data in transit and at rest, access controls limited to authorized clinical and administrative personnel, secure HIPAA-compliant telehealth and electronic health record platforms, business associate agreements with qualified vendors, and ongoing workforce training on privacy and security obligations.
How We May Use and Disclose Your PHI
We may use and disclose your protected health information for the following purposes without your separate written authorization, as permitted by law:

Any use or disclosure of your PHI for purposes other than those described above will require your written authorization, which you may revoke at any time in writing.
Your Rights Under HIPAA
- •Right to Access: inspect and obtain a copy of your medical and billing records.
- •Right to Amend: request an amendment to information you believe is inaccurate or incomplete.
- •Right to an Accounting of Disclosures: request a list of certain disclosures of your PHI.
- •Right to Request Restrictions: ask us to limit how we use or disclose your PHI for treatment, payment, or operations.
- •Right to Confidential Communications: request that we contact you in a specific way or at a specific location.
- •Right to a Paper Copy: receive a paper copy of this notice upon request, even if you have agreed to receive it electronically.
- •Right to Notification of a Breach: be notified in the event of a breach of your unsecured PHI.

Our Responsibilities
Raeven Health is required by law to maintain the privacy and security of your protected health information, provide you with this notice of our legal duties and privacy practices, abide by the terms of the notice currently in effect, and notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
Changes to This Notice
We reserve the right to modify this notice and to make the revised notice effective for all protected health information we maintain. The most current version will always be available on this website and in our office.
Filing a Complaint
If you believe your privacy rights have been violated, you may file a complaint with Raeven Health by contacting our Privacy Officer through our contact page, or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
