Membership Terms

1. Membership Overview

Raeven Health offers concierge care membership programs designed to provide personalized, accessible healthcare services. These Membership Terms govern your participation in any Raeven Health membership program.

2. Eligibility

Membership is available to individuals who complete the enrollment process, pay the applicable membership fees, and meet any clinical eligibility requirements as determined by Dr. Marissa McCarthy. We reserve the right to decline membership at our discretion.

3. Membership Fees

Membership fees are charged on a recurring basis as specified at the time of enrollment. Fees are non-refundable except as expressly provided in these terms or required by law. Fee changes will be communicated with at least 30 days notice.

Membership fees cover the services explicitly outlined in your membership agreement. Additional services, procedures, laboratory tests, imaging, or specialist referrals may incur separate charges.

4. Membership Benefits

Benefits vary by membership tier and may include:

• Direct access to Dr. McCarthy via phone, messaging, or video
• Extended appointment times
• Same-day or next-day appointment availability
• Comprehensive annual physical examinations
• Coordination of specialist care
• Personalized wellness and prevention planning

5. Cancellation

Members may cancel their membership by providing written notice. Cancellation policies, including any notice periods and refund eligibility, are specified in your membership agreement.

6. Insurance

Membership fees are separate from and do not replace health insurance. We do not bill insurance for membership fees. Members are encouraged to maintain comprehensive health insurance coverage for hospitalizations, emergency care, and services outside the scope of the membership.

7. Changes to Terms

Raeven Health reserves the right to modify these Membership Terms at any time. Material changes will be communicated to active members in advance. Continued participation constitutes acceptance of revised terms.
