Telehealth Consent

Introduction

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. This Telehealth Consent explains how telehealth services work at Raeven Health.

Nature of Telehealth Services

Telehealth services may include, but are not limited to, consultation, diagnosis, treatment, follow-up care, and patient education using secure video conferencing, telephone, messaging, or other electronic means.

I understand that telehealth is not appropriate for every medical condition. The healthcare provider will determine whether telehealth is appropriate for my specific needs. If my condition requires an in-person evaluation, I will be informed and directed to seek appropriate care.

Telehealth services are available only where Dr. McCarthy is appropriately licensed or otherwise permitted to practice.

Potential Benefits

I understand that the potential benefits of telehealth include:

  • Improved access to medical care and expertise
  • Convenience and flexibility in scheduling
  • Reduced travel time and associated costs
  • Continuity of care when in-person visits are not feasible

Potential Risks

I understand that the potential risks of telehealth include:

  • Technical failures or interruptions that may affect quality or delivery of care
  • Security breaches or unauthorized access to transmitted information, despite security measures
  • Limited ability to perform certain physical examinations or diagnostic tests remotely
  • Delayed response times due to technical issues
  • In rare cases, lack of visual or audio clarity may lead to incomplete or inaccurate information

Confidentiality & Security

Raeven Health uses secure, encrypted platforms for telehealth communications. However, I understand that no technology is completely secure, and there is always a small risk of data breach. I agree to use a private, secure internet connection and to participate in telehealth sessions from a private location.

Emergency Protocols

I understand that telehealth is not for emergencies. If I experience a medical emergency during a telehealth session, my provider may direct me to call 911 or proceed to the nearest emergency room. I acknowledge that my provider may not be able to provide immediate physical intervention through telehealth.

My Rights

I understand that I have the right to:

  • Decline or withdraw consent for telehealth at any time
  • Request an in-person appointment instead of or in addition to telehealth
  • Access my medical records in accordance with applicable law
  • Be informed of the credentials of the healthcare provider delivering services

By scheduling or participating in a telehealth appointment with Raeven Health, I acknowledge that I have read and understood this Telehealth Consent, have had the opportunity to ask questions, and agree to receive healthcare services via telehealth.
